Filling Hollows vs. Restoring Structure

The Under-Eye Isn’t the Problem

Most under-eye complications don’t come from poor injection technique.

They come from treating the wrong area.

The tear trough is rarely a true volume deficit.

It’s a structural and perceptual issue created by bone resorption, fat pad shift, ligament tethering, and disrupted light flow.

The Natural Trough™ Technique teaches injectors how to treat the cause—not just the hollow.

Treat the Cause. Not the Hollow.

Most injectors are taught to place filler in the trough.

The Natural Trough™ Technique teaches you to step back and ask why the trough exists in the first place.

NTT is an anatomy-driven assessment and treatment philosophy that focuses on restoring orbital support, midface structure, and light continuity—while avoiding high-risk, lymphatic-prone zones.

This is under-eye rejuvenation designed for longevity, safety, and natural outcomes.

What You’ll Learn to Assess

The Natural Trough™ Technique is built on understanding how orbital aging actually occurs—and how improper sequencing creates long-term complications.

Not every under-eye needs filler.

Not every hollow should be corrected directly.

Not every patient is a candidate for tear trough treatment.

NTT teaches you how to:

  • Assess orbital anatomy, ligament tethering, and fat pad position
  • Identify when tear trough concerns are structural vs perceptual
  • Restore support in adjacent zones to reduce trough depth indirectly
  • Avoid hydrophilic filler placement that leads to edema and migration
  • Create refreshed under-eyes that remain natural over time

This approach allows you to achieve brighter, smoother under-eyes without chasing the trough itself.

The Natural Trough™ Philosophy

  • Structure Before Volume

True under-eye aging begins with maxillary and orbital bone loss. Supporting structure first prevents puffiness, migration, and distortion.

  • Light Flow Matters

The eye reads youth through smooth transitions of light—not flatness. Proper support restores reflection without overfilling.

  • Avoid the “Sandwich”

Filler placed between key ligaments traps fluid. NTT teaches where not to inject just as much as where to support.

  • Never Aim for 100% Correction

A slight hollow is natural. Overcorrection creates heaviness and long-term complications.

FAQs

EXPERIENCE REQUIRED

To ensure the highest standards of care and expertise, we require all participants to hold the appropriate credentials. Specifically, you must be a NP, PA, RN, DDS, or MD. Proof of these credentials is mandatory for enrollment in our training programs or shadowing.

All of our in-person trainings are designed for injectors who have been practicing for a few years and are confident in foundational techniques—especially those participating in hands-on sessions. This ensures that each attendee has a solid understanding of facial anatomy and injection fundamentals. The recommended experience level for each course is outlined within its individual description.

what to bring

Just bring your enthusiasm to learn! We provide a workbook, injectables, and all necessary materials. Scrubs or comfortable attire is recommended, as well as something to take notes if preferred.
